Application form

Hello all Mommy Time Mom's

For those of you who were concerned about security and may have tossed out your application, I will email everyone with a new one. Security fraud is a big issue and I'm sorry that it felt too invasive. I'm trying to balance security with trust so that we all feel comfortable sharing child care. Remember, I also want to share my 4 and 5 year old with someone. If you need a new application, print the word attachment ,complete the questionnaire and sign the terms. You will have to mail it back to me.
